Tuxedo- Beat MGS once (Note: Meryl takes over Snake's sneaking suit)
Infinite Ammo Bandana- Beat game with Meryl surviving (Survive Torture Room)
Stealth Camo- Beat game with Meryl dieing (Give up to Ocelot in Torture Room. Note: Otacon will drive escape vehicle instead of Meryl)

Wait a minute here, folks, this is a all wrong now.
1. To get the Tuxeto you have to beat the game twice, and then on the third run, Snake will have a tuxeto and the Ninja will be red.
2. In the Japanese Metal Gear Solid Integral, while Snake has the Tuxeto Meryl will have his sneaking suit. That option is also available on the PC Integral (which is released in US).
3. The whole thing was fixed in MGS The Twin Snakes. On the third run through the game Snake has Tuxeto, the Ninja is red, and Meryl has Snake's sneaking suit.
There, now it's clear.
Note, if you want to get everything, do things in this order:
1st Run: Submit to Ocelot's torture, Meryl will die but you will get the Stealth Suit.
2nd Run: Now, with the help of the Stealth Suit, run through the game the second time, but do not submit to Ocelot's torture. Now you will get the Official Ending, as well as the Infinite Ammo bandanna.
3rd Run: Now you will have the Stealth, the Bandanna, and Snake will have the Tuxeto. Beating the game the third time will change the ending credits from "The Best is Yet to Come" to "MGS Main Theme".
