What you will need:
1 Chaff Grenade
1 C4 Explosive
2 Grenades
As the battle starts Snake is behind a rock. Through a Chaff grenade and the quickly equip a C4. As the Chaff explodes and confuses to the Tank's guidence, run directly at the Tank. The gunner on top will try to shoot, but it's easy to avoid by shifting a bit to the right. As you get close, the Tank will back up a bit. Run to where it was prior to moving and plant a C4 (far more effective than a Claymore Mine). The Tank will roll back in position on top of the C4, that's when you blow it. After that the Tank will barely move.
Quickly toss one grenade at the gunner, that will kill him. Use the few seconds break to aim the second grenade. Once the second gunner shows up, toss the second grenade.
DONE!
NOTE: This strategy does not work for the Tank battle in The Twin Snakes.
