1. The first and best way is to use the Ketchup given to you by Otacon. While the guard is in the bathroom lay on the floor, equip the ketchup and use it. It will look like you are dead in a pool of your own blood. The guard will see it, open the door, run it to examine you. You get up and take him down.
2. If you failed with the Ketchup, you can still escape, but it gets tricky. You can hide under the bed while the guard is in the bathroom. He'll run in and see the cell empty. As he looks around the cell, get out from under the bed and take him down. But this is more time consuming and is harder to get to the guard without him running out of the cell.
3. If you failed time and time again, eventually the Ninja will come in and open the door for you. But beware, you will still have to deal with the guard, who is comming back from the bathroom.
Note: If you submit to the torture, you won't have to be tortured again. But if you keep resisting, but failing to escape, you will be tortured more and more.
