1 - Similar to the ketchup, the Fake Death Pill will phase you into the 'Snake Is Dead' screen. When Johnny Sasaki goes in, use the Revival Pill, and take him down with the Fork.
2 - The second method, is to enter Survival Viewer, go into the Cure Menu and hit R1 for First Person View. Spin Snake around multiple times, then exit the screen and he should throw up. When Johnny Sasaki sees this, he will clean the mess up allowing Snake to flee.
3 - Finally, you can also toss the food he gives you back outside for the guard to eat. He'll rush to the bathroom; use 144.75 on your radio to open the cell door.
Notes:
Do not consider the first two methods if you are going on no alert, no kill run through.
This is a bit dirty, but there is a poster of an almost naked woman inside the guard's room (just as there are other similar posters in Graininy Gorki and Groznyj Grad)
Ignore the wounds that Snake has if you want an extra cutscene behind the waterfall (which shows Eva hilariously removing the transmitter from Snake in a very suggestive method. There is also an extended theatrical teaser displaying Eva fighting the Ocelot troops who guard the waterfall). Be aware however that if you do not take out the transmitter, you will incur a Caution phase when Snake wakes up from the Sorrow's encounter. A markhor that I have noticed will usually be there may disappear if you are in Caution phase too.
I suggest to take care of Snake's wounds when he ends up in the jail so that you will not have to incur Caution, remember, having the transmitter still in Snake will cause a bunch of Ocelots to patrol the waterfall. You can kill the guards with the revolver if you want to (if you have ammunition), but if you are going on no alerts, no kills, it's best to avoid such a situation. Plus with the Naked Camo on, you have little chance of slipping past without the patrols noticing you. P.S. - you will acquire all your equipment except camo when experiencing The Sorrow, but you can't actually kill the ghosts. However, when Snake wakes up, all his weapons and gadgets are gone until he meets up with Eva behind the waterfall. So don't think you can wear the Spirit suit and try to sneak past the Ocelot fellows, you can only acquire this uniform after Eva rendezvous with Snake. Even then, if you drown during The Sorrow experience, then use the Revival Pill, you will not have obtained his Spirit uniform, so I say you actually make it to the end of the river.
