I think, if you look closely, his right eye is white, but I'm not sure.

Nope, I checked. The part when you are under the beds, I went to the same side as him and was next to him. I saw it clearly. The eye was intact and blinking.
This kind of lack of attention to detail is frustrating. Also, if you look closely, he has no facial hair under the bandage. BTW, there is only about 3 hours from the time the ambulance crashes to the time Big Boss is on the motorcycle. In that time he went back to the eyepatch and full facial hair.
It's funny, the more I played old MGS games, the more I found things I liked. Even MGS2. But with MGS5, I just keep finding things that make the experience worse.

Kojima just couldn't be fvcked to give a quarter of a sh!t anymore. In fact, the very act of sacrificing story and attention of details more than speaks for itself. Even if gameplay in MGS5 seems unbeatable, Uncharted and LoU just show how much Kojima has been outdoned.

The gameplay in MGS5 isn't all that amazing. I find that areas are very repetitive and the action is either spamming the tranqs or a firefight. I found that it got old. Also, once you play for a while most guards wear armor and the gameplay gets more frustrating.
Oh, remember how your shadows used to give away your position in MGS2? MGS5 doesn't have that. I tested it.

Yeah, that's exactly what I was pointing out. There are SH!TLOADS of better games out there that embarass Kojima. Not too sure why Konami would keep him with their studios. As we ALL know, Naughty Dog puts MGS to shame.
And why the fvck should I get MGS5 even though it seems fun, when we have Watchdogs comparing with GTA or Saints Row out there.

I can't vouch for other open world games since I haven't played them. I'm just comparing MGS5 to other MGS titles. Sure, the areas are much bigger, but the action isn't. The paths between enemy outposts are huge but very empty. The first few times its cool to ride a horse and enjoy the view, but then it gets old. And so do the enemy basis. With the exception of few, most are very similar. I look at MGS3 and MGS4 where the maps are smaller, but much more developed. Also, both games had a good variation of environments. MGS3 had jungles, swamps, mountains caves, and interior settings. All 5 acts of MGS4 offered a very different setting and feel. In MGS5 there is little difference between the Afganistan and Africa. And equally little to do. There is no interior settings. Just two giant maps with repetitive outposts.
Again, I can't compare this to other open world games, but that's not my goal. I don't care about other games. I care about Metal Gear. MGS5 sacrificed a lot of what I love about the series in favor of attempting mechanics they weren't familiar with. The end result is a very empty game. Seriously, you should be able to do more at Mother Base than target practice, shower, and look for diamonds. It's a huge area with nothing to do. Seems like missed opportunity.
Having said that, I actually like Ground Zeroes much more. The map was very well put together. If they added some interior settings to the the admin building and perhaps a boss fight I would say it's a good package.

In MGS3 and 4, Kojima succeeded in making small maps fvcking interactive. But in 5, as mentioned above, we get giant maps that lack any sort of activity or wildlife. I guess the decline of MG is further evidenced by the story generally making no sense with the completion of 4.

Actually my favorite part of DarkSydePhil's playthrough is when he decides it's a good idea to go after the wildlife and tries to take down a bear.

No doubt MGS5 has wildlife. But...alas, even MGS3 had SH!TLOADS of animals and plants compared to the Phantom Payne.

Just two giant maps with repetitive outposts. i can, I've played the likes of Assassin's Creed and The Witcher 3 (my personal GOTY btw. especially if you want a good story and good gameplay, even if you haven't played 1 and 2 or don't play western RPGs in general, I definitely recommend it). And true, the game world in MGSV is quite empty. But I don't think MGSV is supposed to be a vivid open world, but more of a giant sandbox environment for all kind of gameplay concerning infiltrating and exfiltrating. The animals and plants to collect are just bonus collectibles on the side. Like flags/feathers or hidden packages in AC or GTA. Mother Base is definitely way too empty. At least a few more interior environments would have been great. Also, I remember in last years E3 gameplay demo, you could find Ocelot and Kaz walking on the base and interact with them. I miss that in the final game. Also, I found it strange, whenever major characters were in a cutscene on MB and that cutscene was about to end, Venom always smokes the Phantom cigar and skips to a point in time when all these characters are hidden in their cabins again. WTF? And the whole Battle Gear business. Maybe that is the point of calling the game "The Phantom Pain", because it's so obvious so many elements that were planned are missing.

I do indeed think Kojima called it Phantom Pain since he wanted to literally give players a sort of 'phantom pain'. Y'know, 'cos the story was disappointing and even its gameplay required significant polishing.

Was just playing the Truth chapter. This is still so poorly done! Big Boss clearly has two eyes and no facial hair. You can see that where the bandages aren’t covering his face. That’s at 2AM when they are escaping the hospital. 3 hours later he is talking with Ocelot. Full beard. Also, why was his face bandaged up? Based on the passport Ocelot gave him it would be logical to have him with his new face. Which would explain why he looked different under the bandages. And yet he still has his old face.
Just terrible. I love the hospital chapter but that’s just bad storytelling.
